Summary: CSV Reader and Writer services should assume there is no
header by default

As it is currently set it assumes the first line is a header by default which
could result in data loss if not configured properly for the dataset. We
should have a default which would cause the header line to get processed in the
worst case rather than the first real record lost in the worst case.
