However, since the Export... -> XHTML function has been removed from OO 4.0.1, I can't test that the ToC alone is enough to reproduce the problem. If I "save as HTML" this file the result is OK. When I filed this bug a long time ago, "Save as HTML" on the full document had even more problems than Export.... My goal is to create an HTML version of the document for distribution not an HTML version that can be used for continued editing. Save as HTML seems to be for the latter purpose.
