I observed that problem for own attachments (see
<https://issues.apache.org/ooo/show_activity.cgi?id=123246>) and for
attachments of other very experienced users where I am pretty sure that reason
was not a mishap.
Steps how to reproduce: Simply attach a WriterDocument.odt without touching the
mime type selection radio buttons (Default=Autp)
