Reproduce step:
1) New writer document and input two paragraphs.
2) Press F11 to open Styles dialog
3) Assign the first paragraph to Heading 1 style and the second Heading 2
style.
4) From main menu, click 'Insert-)Index and Tables-)Indexes and Tables...' to
pop-up 'Insert Index/Table' dialog.
5) Click OK button
Defect: a 'General Error' message will popup. But there's no problem on Windows
