@ marcus,the file "Untitled 1" was not empty! 

Sorry but last night, I reproduced only on MacOS X, 
and I lose the contents of the "Untitled 1" file,
a New document overwrites the previous one, instead of displaying a new window
with the title "Untitled 2".

MacOsX 10.9.4 with AOO 411m1(Build-9770) - Rev.1603804

With this version and only with OsX, it only occurs from the opening from a
template:
To reproduce: (needs to have a registered template)
1.Menu : File > New > Template and Documents >  Open an Template
2.Enter data and Save as... "MyDocument"
3.Close the file
4.Menu : File > New > Template and Documents >  Open the same Template
5. In "Untitle 1" Enter data, not save
6.Menu : File > Recent Documents, open the last file.
This file overwrites the "Untitle 1"

This morning restarting Mac OsX, impossible to reproduce!
I think it is more of a saturation OSX 10.9.4 that AOO ...
( RAM : 8 Go and 16 Go : same behavior) 

The workaround is obvious: Restart the Mac !
