First test:
1 - Opened the attachment/document – there were 2 side-by-side Frames, next 2
tables, a single frame and a third table.  This encompassed 12 pages.

2 - Deleted the picture at the end of the document.  (as reported by the
submitter, this step caused the formatting to be corrected) … this did not
change the formatting for me

3 - Deleted the 2 figure boxes at the top of the document.  Upon their removal,
the document did reformat from 12 pages to 4 with no more split tables


Second test:
1 - Opened a new OpenOffice document
2 - From the original attachment/document I copied the entire contents (12
pages) and pasted into my newly created document.  The formatting was
immediately restored – only 4 pages were required to house the original
document contents

Third test (attempt to replicate the initial issue):
1 - After several attempts at creating a document with the same information as
supplied in the initial attachment by the submitter, I was not able to
replicate the issue.  

I plan to evaluate the properties of the tables and formatting in the original
document and report back my findings.  [i.e.: There are frames along with
header types contained in the document.  These should be duplicated to help pin
down (or eliminate) the cause of the table separations]
