Updated information:

After testing this more I have noticed the following:
- This issue is not unrecoverable.
- I get a black screen whenever I close a document that is maximised if that is
the only OO document I have open.
- The Menubars are present, but I need to set my pointer at top of screen to
make it visible. From that menu I can create a new doc and everything is fine.

Summary: This is a more general bug. Closing the last document when in
maximised window causes a black screen which looks like a crash. 

Still testing on same system:
OS X:           El Capitan 10.11.3.
Open Office:    4.1.2 for OS X (AOO412m3(Build:9782)  -  Rev. 1709696
                2015-10-21 09:36:46 (Wed, 21 Oct 2015)
Microsoft Word: for Mac 2011 14.16.0
