I was able to create a form and create some entries in the table. I was able to create a query that successfully produced a report from the table. I used Apache OpenOffice 4.1.2 on Windows 10 Pro with 32-bit Java JRE 1.8.0_92 selected for AOO to use. There need to be more details of the table design and query design to identify the conditions under which the error report was produced.
