I confirm that both AOO 4.1.2 and Libre Office 5.0.0 fail to open the document
on Windows 10 Pro x64.

The .odt document opens successfully in Microsoft Office Word 2016.  It is
presented as a 12 page document that describes a medical research protocol to
be conducted with laboratory animals, for submission to an animal-use protocol
review committee.

There are some highlighted areas and at least one comment from a reviewer.  It
appears that the reviewer created the .odt.

Also, the page footer and perhaps a first-page header carries an image and also
the "Page n of 12" likely auto-numbering. 

(The submitter for this issue is identified as one of the principal
investigators of the research.)

While identification of the defect and problem this proposes for
interoperability using OpenDocument Format (.odt), a short-term workaround is
probably best for the users of the document.

ADDITIONAL TEST RESULTS

Using Microsoft Word 2016 to save the document as a .doc and as a .docx appears
to succeed although the pagination is not preserved exactly.

The .doc opens successfully in AOO 4.1.2.  I have not examined the details but
there are no obvious gross defects.  The .docx opens but is not usable, with
serious formatting problems.
