I was able to reproduce the issue with maximizing to Full Screen by following
the steps described in the original post. On a New Document use: 

* (shift+command+j) on MacOS (10.14.1) with OpenOffice 4.1.5, 
* (shift+ctrl+j) on Linux (Fedora release 28) with OpenOffice
AOO420m1(Build:9800)  -  Rev. 1846402).
* (shift+command+j) on MacOS (10.14.1) with LibreOffice 6.1.1.2

OpenOffice Help (attachment OO_FullScreen_help.png) documents this behavior as
intended. 

On MacOS (10.14.1) with OpenOffice 4.1.0 and 4.1.5, I was able to maximize the
application screen by clicking on the top left corner and choosing the maximize
button (attachment maximize_button.png). This method preserves the toolbars and
menus. Minimize button (attachment minimize_button.png) returns the window to
original size.

I wasn't able to reproduce with 4.0 because only 4.1.0 was available for
download with MacOS (10.14.1).
