I upgraded to 4.1.6, and it's still occurring and is incredibly annoying. I will be working on a presentation, and have inserted numerous pictures and then after a while some of the pictures will disappear and just leave an empty image box. I can't even delete the empty image box, and end up having to delete the slide and redo it. Again, this is incredibly annoying.
