Summary: Rotation of subordinate CA certificates

When rotating a subordinate CA certificate, the following has to be done:
- ensure that there is no in-flight certificate signing
- reset the CA with the new certificate
- remove the old subordinate CA certificate
- allow any new CSR to go through
