Summary: Log EC Replica details if a block cannot be read during

We have seen some issues where ec recontruction fails to complete due to what
looks like bad data in the blocks in some of the replicas. When this happens,
it is very difficult to debug the problem as the logs only container an error.
To help with future debugging of these problems this change will log out
details of the block group, the replica details and chunks and chunk lengths.
Having this information along with the error should allow us to find the root
cause of the failures we have seen.
