Interestingly the patch was not applied to 1.3 branches, however the problem
exists on 1.3 too(slight variation of the one in the jira). Basically if we
have a null covered column, the filter prematurely declares done and skips
generating index updates for other covered columns. Let me put up a patch with
a test that deals with this issue and backport the fix which fixes this too.
