Review Comment:
   Sorry, I guess we're not understanding each other here :sweat_smile: 
   
   I believe page size should not be part of `PageToken` at all. Instead, let's 
keep it in `PageRequest`.
   
   From a higher level view: a client gets some page token, after that the 
client is able to request the next page of any size. The next page size is not 
restricted by the token from the previous response.
   
   I'd like to have a similar delineation of concerns on the server side too.
