Review Comment:
   If the core index is not encrypted (cleartext), then the OutputStream opened 
is not an EncryptingOutputStream. Here we check that to know if the file 
offsets need some twiddling.
   I also considered using a boolean field (instead of instanceof) to indicate 
if the last opened output stream is encrypted. But it does not work inside the 
super constructor call, because the field cannot be set yet.
