Meanwhile want to comment regarding first item. Yes we are using 
{{"constraints": [["hostname", "UNIQUE"]]}}. However here is perfectly valid 
scenario when we are talking about large production cluster: node put into 
maintenance 
mode([http://mesos.apache.org/documentation/latest/endpoints/master/maintenance/schedule/)]
 e.g. for some hardware replacement etc.

Then, when maintenance done, we mark it as "up". At this point offers might be 
accepted by framework, and by marathon, but the order is not defined, i.e. it 
may happen that framework will accept offer before marathon accepted offer, so 
executor will be scheduled before marathon task of external shuffle service is 
scheduled. Eventually the later will be up & running, but meanwhile driver 
failed to connect to it and become inactive.
