Resolution: Implemented
A new ContentTypeHandler is added for x-www-form-urlencoded and the
ContentTypeHandlerManager's functionality split to get a handler for the
request and response separately. The ContentTypeHandlerManager also allows
handlers to be registered by mime-type and/or extension.
