index.jsp from Struts-2.3.4 distro located at
$STRUTS2_HOME/src/apps/processing- forms/src/main/webapp/example/Index.jsp
is incorrect
the updated index.jsp from johannes needs to be copied to
$STRUTS2_HOME/src/apps/processing- forms/src/main/webapp/example/Index.jsp
Johannes updated index.jsp is located at:
https://github.com/apache/struts-examples/blob/master/exception-handling/src/main/webapp/index.jsp
