1. Go to Customize Toolbar.
2. Click Add... on the "Customize" dialog
3. Drag a button from the "Commands" section of the "Add Commands" dialog box
to the "Commands" section of the "Customize" dialog.
4. The button will appear to be successfully added, but then open office
freezes until forcefully killed.
5. When restarting office the drag and dropped button is not actually added.
