A document opened with one template and reopens with another.

I have defined a default template with 'first page' with header and 'default
page' without header but otherwise the same.  If I open a new document using
this it displays as I created it, but if I save it and reopen it the first page
comes up as 'default' and the second page as 'Convert 1', with a different
header and footer.

I attach a document this happened with.
