Update docs for SDK changes

We've made a number of changes to the SDK. Best way of detecting this is to
diff ts/ts.h.in with a version from ATS v2.1.5 (which is pre all these
changes). Also, the apichecker.pl script can help gain some insight on what has
changed, it's available in
