refers to the proxy.config.url_remap.default_to_server_pac setting but doesn't
explain it well.
The Explicit Proxy Caching documentation
<http://trafficserver.apache.org/docs/trunk/admin/explicit-proxy-caching/index.en.html>
does a better job, but doesn't really explain how the corresponding
remap.config settings work and why you might use them.
