Well, what you see is not what I see (I get "-"), but it suspiciously similar.
For me, changing it to log the mapped URL "fixes" it, such that requests served
by the server intercept plugins.
If you could test disabling the plugin, it'd be great, but I don't know if
it'll tell us much (I'm not certain at all that you are triggering the same
problem I am).
