Currently my research indicates the problem is that we should use the 
terminate_sm flag but it doesn't work. There is a crash involving the cache_sm 
in the state machine, which gets an event after the state machine has been 
destroyed. I cannot find how to cancel that event, which should be done in 
HttpSM::kill_this.
