    Description: 
Currently the ESI plugin (with the first byte flush feature turned on) will 
wait for the ESI document to be completely received in ATS before the parsing 
begins and then async fetches for the ESI includes will be made.

The improvement is to start the parsing even before the the complete document 
is received. Thus the ESI includes/async fetches can be made even much earlier 
(i.e. before the complete document is received)
