TS returns PROTOCOL_ERROR when concurrent stream limit is exceeded. ( Both of
PROTOCOL_ERROR and REFUSED_STREAM is allowed as error code in such situation )
It is better to return REFUSED_STREAM error code to support [Request
Reliability Mechanisms in HTTP/2 described in RFC 7540
