Currently, you can't - unless you want to build the form from
scratch each time.
I am working on adding the necessary framework to iText to
allow this and other types of PDF manipulation. But it won't be done
for a bit...

This is a known issue with how Acro5 treats forms differently
than Acro4 - most especially in the area of form field appearances.
