No, you aren't doing anything wrong.
iText, currently (until I get my act together and submit my
patches!), doesn't support "real page copying" - it simply transfers
content between documents.
HOWEVER, even once the page copying patches are submitted,
there are STILL a number of issues present when trying to concat
documents with form fields - ESPECIALLY if the fields have the same
names in each documents. This is one reason why "form flattening"
was invented.
There are some commercial products that can do what you want...
