It depends. If you want a generic solutions then the option is to convert
each pdf page to an image (with gs) and then make the white transparent. If
you have many documents in the same situation and they all have the same
structure then go to the image dictionary and make white transparent. Either
way, deep knowledge of the pdf specs is always required as this is really a
fix to a "bad" pdf.
