After adding these lines, it works!

    response.setHeader("Pragma", "public");

I also encounter a new issue with image

    // create and add the letter logo for testing
    String newUrlStr = "https://www.xyz.com/common/images/phsLogo.jpg";;
    URL imageUrl = new URL( newUrlStr );
    Image letterLogo = Image.getInstance(imageUrl);
    document.add(letterLogo);

With the above code for adding an image into a document, now I see a blank PDF page. 
Remove that those image lines, then it works fine. Though, I can view that image by 
pointing that URL on my browser. Do you know what I'm missing?
