It's a bug. The determinant should have been used instead of the x
dimension. Replace in PdfGraphics2D line 458:
float scale = Math.abs((float)transform.getScaleX());
with:
float scale = (float)Math.sqrt(Math.abs(transform.getDeterminant()));
