Forget about Watermark, it only gets in the way. Do something like this:

Image img = Image.getInstance(...); // make it accessible to the page event
img.setAbsolutePosition(0, 0); // or other value
img.scalePercent(100); // or whatever

public void onStartPage(PdfWriter writer,Document document) {
    writer.getDirectContentUnder().addImage(img);
}
