You don't need to access the pages in random order. If you have all the
image dimensions you knoe what goes where before starting to output the
images. If you really need random access you'll have to place the images
in templates that will act has pages.

PdfContentByte.addImage(). The rest is a matter of 1+1.
See the javadocs for Image. There are examples of using images ao
itextpdf.sf.net and at the tutorial specially for the unit and matrix
explanation.
