A Postscript backend is my favorite backend for a itext wish list. Parts of it should be really easy because of similarities between pdf and postscript but others are very hard to implement like transparency flattening if you like to implement similar features in both outputs.
This is difficult to answer, even the PDF support is not *complete*.
But many of the unsupported features are not so important. And even a simple postscript backend could buy you important possibilities like input/output paper tray switching, jogging or stapling that are in many workflows not possible using pdf or rtf.
