I figured out the two pass way! I simply run through once with a shiftPageNum=0 (shift the page number of all pages after the toc, and save how many toc pages there were in the pdf. Then run it through a second time changing the shiftPageNum to the number of toc pages found in the first run. Works great! Thanks :)

Are you making your ToC using page reordering? Maybe you could generate your actual document in one ByteArrayOutputStream, your ToC in another one, then use PdfCopy to concat both and PdfStamper to number the pages.
