That's the File Attachment Annotation - it's but one way to
embed files in a PDF. The other is to simply embed it - use the
Attach File command in Acrobat.

Via pdfmaker in outlook it gave me html-style hyperlinks to embedded
attachments.
Yes, those are the types of annotations that I am talking about.
