Well, the original PDF documents were created using
the Acrobat editor and not programmatically.  So when
we first spotted the problem, I assumed a field name
and/or other internal properties were duplicated,
causing checkbox/text fields not to work as desired.

I guess my question now is, is there something in the
library that'll be able to read through a document and
uniquely rename these checkbox/text fields?
