If you want to go through all that you might as well program in
assembler to make it even more difficult. I'm a little lost about the
original question but:

- if you want to create a new appearance call
PdfContentByte.getAppearance() and put inside  whatever you want that
the resouces will be created and so on. When the doc is closed the
appearance will be written to the output stream. To set the appearances
use PdfAnnotation.setAppearance().

- if you want to import an annotation the work is more or less done. See
the source code of PdfStamperImp.addComments().
