I don't think there's an easy way as Top.Middle.Lower
is a constructed name. The field with name Lower is the
kid of a field with name Middle which has a field with
name Top as parent.
You would need to retrieve all the field information
(this is possible with iText), then flatten the file (possible),
then construct new fields/annotations and add them to
the PDF (possible with iText).
I don't think iText does this by default if you use
renameField.
