That's what I figured... But how do I get the dictionaries? (Catalog and
MarkInfo are the two giving me most trouble at the moment. I have learned
how to create the required dictionaries but can't figure out how to get the
"parent".

         I don't believe it's implemented by iText at this time, but 
using the existing code for doin a FileAttachment annotation - it 
won't be difficult to do an "named attachment".  They are the same 
data structure (embedded file /EF) but live in the /Catalog/Names 
dictionary.  Check the PDF Ref for details.
