Acrobat 7 can create AcroForms; make sure Adobe LiveCycle doesn't
start, just add annotations (text field, button,... annotations).

You could use iText to create the AcroForm, but only if you want
to do it programmatically. If you want a GUI, you would need
a tool like JPedal in combination with iText.

These are the 2 STEPS that I have to implement:
STEP 1.

on the Client side (like a template) with a designer tool and then
upload it to server side.
OK. See above, this can be done with Acrobat.
STEP 2.
I'll use iText to prefill some of the template fields on the fly.
