Ouch. We downloaded the latest 1.4.1 iText and we're still seeing the
error.
What we are doing is reading in PDF's and then assembling a new PDF that
contains all the small chunks of pages, and then adding in TOC and page
links.
Basically with the streaming methods any kind of embedded XML seems to
be the kiss-of-death - and we get various errors and iText fails to
process the PDF completely. It will process the PDF up to it encounters
the XML and then it throws an error and stops - or it appears to process
OK - but when you look at the final resulting output its missing the
pages starting from where it first encounter XML in the original PDF.

The XML that you included is XMP
metadata and has been part of the PDF spec since
Acrobat 5 (PDF 1.4). iText has supported both
READING and WRITING of XMP for at least a year.
What sort of problems are you having?
