The word 'template' can be used in many different contexts.
You will have to clarify what you want to do.

Do you have an existing PDF file with a form (AcroForm with AcroFields);
then you can use iText to fill the fields of the form.

Do you have an existing PDF file without a form,
then you can use pages from this PDF document as background
for a new PDF and stamp extra content on it.
We use this frequently if we want to send standard letters
on which we stamp addresses, barcodes, and so forth.

Please explain.
