I would thus speculate that there is some problem accessing the images. Have you tried writing the document to pdf instead of to rtf to determine if the images are being handled at all? Does the server have the permissions to access the images? Possibly this is also a problem that existed in 1.02b, but has been fixed in a recent revision. Try seeing if upgrading solves your problem.
