OK, I realize everyone is busy so that's probably why I didn't get a
response yesterday.  So, I spent all day today trying some things out.
I was able to finally load a checkbox into a PdfPCell and I was able to
load a dash line into a PdfPCell.  I'm not sure if I did it the best way
but it worked.  I tried the setLineDash but I couldn't get it to work,

 

Now, a horror of all horrors, after I checked the checkbox and printed
the form, the checkbox is unchecked on the printout.  I am using version
7.0 of Acrobat.  I tried saving the form and then printing it and the
box was still unchecked.  

 

I remember that a checkbox can be used to take an action.  Can this
action be to set the checkbox somehow so it will print and save
properly?  We want to use these pdf documents to allow our customers to
select certain items from a pdf form and return the form as an email
attachment so we can see which boxes they checked.

 

I have attached the java source and the output pdf.
