OK, figured this out. It is a 2 step process: 1) Concatenate forms by using PdfCopyFields. After this step the submit button will be not functional yet.
2) Now use PdfStamper (as mentioned in Section 18.4.2) of the book to change the URL; except instead of modifying the URL, create a new PdfDictionary using PdfAction.createSubmitForm and set it to the submit button.
