Either you'll have to find another product that can turn
HTML forms into a PDF form (I don't know any; I don't think
Flying Saucer is able to deal with formsmaybe you could
open the HTML in OpenOffice and save it as PDF).

Or you'll somehow have to adapt the iText code so that
form specific HTML tags are translated to a PDF form.
If that was an easy job, we'd already have done it,
but it's not easy and it's your urgent priority, not ours.
