My first reaction is: on what image? Are you talking about an image in a PDF file? If not, you're on the wrong mailing list. Do you know the position of the image in the PDF file? Do you know where to put the text? If not, you'll have a hard time achieving what you want (with any product, this isn't a problem limited to iText). If you do know the coordinates where you want the text, then it's easy.
