Not possible with PDF at all (let alone iText).
Give your client 2 PDF files - one for double sided printing, one without. I'll leave inserting blank pages as an exercise to the reader :-)

I am using the wonderful iText library to merge some pdfs in one of my projects and so far its working great. We generate a bunch of pdfs as the output of one of our business processes, and then we combine it into one pdf for the user's convenience. My client has made a request now which has me wondering if its even possible. When I merge the individual pdfs, if one of them is an odd-number of pages and he prints it out duplex (double-sided), the first page of one of the sub-documents ends up on the back of a piece of paper which is not what he wants. I suggested that I could just put in a blank page if the document had an uneven number of pages (even though I'm not sure exactly how to do that, pointers appreciated!), but he said that it should not print blank pages when printed without duplex (when printed single-sided in other words).
